We are pleased to have you as a new patient in our practice! Whether you are joining us to address a specific concern or for routine dental care, we are committed to helping you establish and maintain a healthy mouth. We believe in patient education and preventative dentistry, following up-to-date standards of care. In order to save some time in the office we would like to provide you with some information about the practice and ask that you give us some information about you and your family with the forms below.

On your first visit we will provide you with a comprehensive examination that includes a thorough review of your medical and dental history, any necessary radiographs, an oral cancer screening, and a complete evaluation of your teeth and gums. If you should be seeing us for an urgent concern we may limit our exam to the information needed to address your discomfort and then have you return for a more comprehensive exam at a later date.

Using the information obtained from the clinical examination, medical and dental history, and any other necessary diagnostic procedures, a suggested plan of treatment will be developed. We can then discuss the pros and cons of various treatment options, explaining the various procedures involved. We will provide you with an estimation of your out of pocket cost. We work with many different insurance companies and will do our best to provide you with an accurate estimation of their coverage and to assist you by filing the insurance claims. Please be aware that our advice, recommendations and treatment are for you the patient and although we will do our best to work with your insurance company, the charges are ultimately your responsibility. If you have insurance, we ask that you pay your estimated portion on the day of treatment. If you do not have insurance, we ask that you pay for all services in full.
